CONFIDENTIALITY:
All information is confidential and that confidence is strictly upheld. Personal information may be accessed or reviewed with
clinical or administrative staff employed or contracted by Anderson Orr. Anyone associated with this practice with access
to your personal information is bound by law and ethical duty to protect your confidentiality. No information on your presence
or participation here will be disclosed without your written authorization to do so. However, counselors and/or therapists
are bound by law and ethical duty to the following exceptions to client confidentiality: - Suspected child
and elder abuse and/or neglect must be reported.
- When a person is in clear and imminent danger of harming themselves
or others.
- When a court order mandates the release of information.
- Medical emergencies (when a person becomes
physically unable to request assistance)
Email and Text Disclaimer: If you desire to communicate via email and/or
text I am willing to do so. However, email correspondence is usually not a secure medium and therefore poses increased
risk to one's confidentiality. Emails and texts, even when "deleted," remain on computers and servers
and are capable of being retrieved unless properly erased/overwritten. APPOINTMENTS: Services are provided by scheduled
appointment only. I require one working day notice for cancellations. Please call as soon as possible if you need to reschedule.
Failure to give a 24 hour notice may result in you being charged for a “no show” or late cancellation fee. My
general policy is that I will not available "after hours," weekends, or holidays -- but you are welcome
to leave a voice mail or send an email and I will respond if/when I'm available. Therefore: In
case of an immediate mental health/ medical emergency situation — please call Holly Hill Hospital (919-250-7000)
or Wake County Mental Health Crises & Assessment Services (919-250-1500), go to a hospital emergency room, or call 911. Please,
do not arrive for a session under the influence/impaired by alcohol or other mind/mood altering substances. If it seems you
are under the influence you may be refused your appointment, not allowed to operate a vehicle, and be required to have someone
pick you up. In addition, you may be charged for the session as well as any other time required to help resolve the situation. FINANCIAL
POLICY: Payment is expected at the time of service unless prior alternative arrangements have been made. At this time, I do
not file insurance but you may be able to submit claims for "out of network" reimbursement (according
to your policy benefits) with a statement of services and fees I can provide for you. In some cases,
my fees may be similar to one's insurance “out-of-pocket” deductible and co-pay expenses. You may make payments
by cash, credit card, or checks payable to Anderson Orr, M.Ed., LCAS. There is a $25 fee for returned checks.
Please do not request verbal confirmation or written documentation of services and/or progress be provided to non-clinical
parties unless fees are current and you have signed appropriate authorization to release information forms. Standard
self-pay rates for services include: Individual Counseling (50-60 minutes): $95. Initial
Consultation or Clinical Substance Abuse Assessment (1.5 hr.): $142. Brief Consultation or Counseling Session
(30 minutes): $60. 1.5 hr. Educational Class (group): $42. each. 1.5
hr. ABC Group Counseling Session: $42. each. Detailed reports or letters requested by client: $25-$35. Urine
Drug Screens: $25-$35 each (depends on type of drug panel) Appointment NO SHOW fee = Session Fee. Late
Cancellation Fee (less than 24 hrs) first occurence = $60.
